Contact Me Widget

Feature Description:  Create a customizable form to collect information from your site visitors. The form can be added to any page or sidebar area. The form can be set to launch as a modal window from a button click or embedded on a page itself. When the visitors complete the form, the email address(es) set up to be notified will be emailed the form response. 

 

This was built specifically to address the need of collecting information about visitors to Exhibit hall and booth pages. However, this tool could be used in other circumstances, such as end user support or a way to submit ideas for new learning content.

Upon click, the modal opens with the form

 

 

Sample form response email. The form will be sent from the form submitter and allows for direct reply to the submission email

 

Feature Admin Settings:  

** *this feature will need to be activated on each site. To get this activated, email elevate-support@gocadmium.com

Under Content Management, Pages or Sidebar, add a new widget, select Contact Me Widget

 

Then customize your form fields, click the field label to edit or to reorder by dragging and dropping. Click the form field options along the right hand side to add additional fields.
